Office of Vital Statistics Services

The Office of Vital Statistics maintains birth and death records for the City of Portsmouth, with records going back as far as 1909.

A copy of a record can be obtained for the following charges:

• $25.00 - Certified copy

Certified copies must be used for marriage licenses, passports, school registration, driver's license, traveling out of country, social security, state I.D., insurance purposes and court purposes.

To request a copy of a record, mail the following to Portsmouth City Health Department, 605 Washington Street, Portsmouth, Ohio 45662:

• A $25.00 money order made payable to the Portsmouth City Health Department
• Self-addressed stamped envelope (an overnight service can be used if the record is needed quickly)
• Signed request including the full name at birth or full name at death, the birth or death date, parents' name (including mother's maiden name), and a phone number where you can be reached in case of a problem
